The answer is reassuring: in many cases, only a part of the liver is sufficient for a successful liver transplantation. This is possible because the liver is a unique organ with a remarkable ability to regenerate and grow after transplantation.

Understanding Liver Transplantation

A liver transplant is a surgical procedure in which a diseased or failing liver is replaced with a healthy liver or a portion of a healthy liver from a donor.

Depending on the source of the donor organ, the transplant may involve either a whole liver or only a part of the liver.

Types of Liver Transplantation

Living Donor Liver Transplantation

In a living donor liver transplant, a healthy individual donates a portion of their liver to the recipient.

Only a segment of the donor’s liver is removed and transplanted. The remaining liver in the donor regenerates and grows back over time, allowing the donor to continue living a healthy life with normal liver function.

This type of transplant is particularly valuable because it reduces waiting time for patients with end-stage liver disease.

Split Liver Transplantation

In certain deceased donor cases, a single donor liver can be divided into two portions and transplanted into two different recipients.

This procedure, known as a split liver transplant, allows one donor organ to save multiple lives. Typically, one portion is transplanted into a child while the larger portion is transplanted into an adult recipient.

Deceased Donor (Cadaver) Liver Transplantation

In adult deceased donor liver transplantation, the entire donor liver is usually transplanted into the recipient.

Since the liver comes from a deceased donor, the whole organ can be used when appropriate, especially for adult recipients requiring a full-sized liver.

How Can a Partial Liver Be Enough?

The liver is one of the few organs in the human body capable of significant regeneration.

After transplantation, the transplanted liver segment begins to grow rapidly during the initial weeks and months. Over time, the growth rate slows, and the liver gradually reaches a size that adequately meets the recipient’s metabolic and physiological needs.

This regenerative capacity is what makes living donor liver transplantation possible and successful.

Is There a Minimum Liver Size Required?

Although a partial liver can function effectively, it must be large enough to support the recipient’s body requirements.

One of the important measurements used by transplant surgeons is the Graft-to-Recipient Weight Ratio (GRWR).

As a general guideline:

  • The graft weight should be approximately 1% of the recipient’s body weight
  • A recipient weighing 60 kg typically requires about 600 grams of liver tissue
  • A GRWR of around 1% is considered ideal for successful transplantation

This ensures that the transplanted liver can adequately perform essential functions such as:

  • Detoxification
  • Protein synthesis
  • Bile production
  • Metabolic regulation
  • Blood clotting factor production

What Happens If the Liver Graft Is Too Small?

When the transplanted liver segment is significantly smaller than required, it may not be able to meet the body’s metabolic demands.

Generally, if the graft size falls below approximately 0.8% of the recipient’s body weight, the risk of complications increases.

For example:

  • A 60 kg recipient ideally requires about 600 grams of liver tissue
  • A graft weighing less than 480 grams may be insufficient

In such cases, the recipient may develop a condition known as Small-for-Size Syndrome, where the transplanted liver struggles to perform its normal functions effectively.

Symptoms may include:

  • Persistent jaundice
  • Fluid accumulation in the abdomen
  • Poor liver function
  • Delayed recovery
  • Features of liver failure

For this reason, careful donor and recipient matching is a critical part of transplant planning.
